The program
Primary care for a predictable monthly or annual payment
The EFMD Direct Primary Care Membership is a self-pay primary care program for patients who are appropriately treated as self-pay at Enrique Fernandez MD PA. This includes patients without health insurance and, when permitted after Practice verification, patients with commercial coverage in which the Practice does not participate.
The EFMD Direct Primary Care Membership is not health insurance and is not a substitute for health insurance. It covers only the primary care services specifically described in the membership agreement. Patients should consider maintaining appropriate health coverage for services and expenses outside the membership.
What you get
Your membership includes
Annual exam
One annual comprehensive/preventive examination.
Four more visits
Up to four additional in-office primary care visits per membership year for acute care, follow-up, or chronic disease management.
Medications
Medication management and routine prescription refills.
Results review
Review and interpretation of laboratory results, imaging reports, and specialist reports.
Care planning
Preventive counseling, care planning, referral coordination, and clinically indicated routine ECG.
Healow access
Secure electronic access to available health information through Healow.
Unused included visits do not accumulate or carry forward into a subsequent membership year.
Telehealth visits are not included. The membership is designed around in-office primary care.
Pricing
Simple, predictable pricing
Billed each month. A 10-calendar-day grace period applies if a payment fails.

Additional in-office primary care visits during the same membership year are $50 per visit.

Additional medically indicated laboratory testing may be available at the practice's negotiated discounted self-pay laboratory rates. Pricing depends on the specific tests ordered.

Membership & cost
What is the EFMD Direct Primary Care Membership?
A direct primary care arrangement for specified primary care services from Enrique Fernandez MD PA for a predictable monthly or annual payment.
How much does membership cost?
$99 per month or $1,050 for a 12-month annual prepayment.
When does my membership year begin?
On the Effective Date shown in your enrollment documents and continues for 12 months.
What happens if my monthly payment fails?
You have a 10-calendar-day grace period to correct the failed payment. The grace period does not change your original membership or renewal dates.
Can I cancel?
Yes. Either party may terminate under the written notice provisions of the Agreement, generally with at least 30 days' advance written notice, subject to the Agreement's immediate-termination provisions for breach or violation of the physician-patient relationship.
Visits & services
How many visits are included?
One annual comprehensive/preventive examination plus up to four additional in-office primary care visits during each 12-month membership period.
What happens after I use the four additional visits?
You remain a member. Additional in-office primary care visits during that membership year are $50 each.
Do unused visits carry over?
No. They do not accumulate, transfer, or carry forward and have no cash value.
What else is included?
Routine medication management and prescription refills, review of laboratory/imaging/specialist reports, preventive counseling and care planning, referral coordination, clinically indicated routine ECG, and electronic access to available health information through Healow.
Are telehealth visits included?
No. The membership is designed around in-office primary care.
Are annual labs included?
No. The annual preventive laboratory panel is $230 and currently includes CBC, CMP, lipid panel, Hemoglobin A1c, TSH, and urinalysis.
What if I need other labs?
Additional medically indicated testing may be available at negotiated discounted self-pay laboratory rates. The price depends on the tests ordered.
Can I access my records electronically?
Yes. Members have secure electronic access to available health information through Healow.
Does membership guarantee immediate appointments?
No. Appointments remain subject to clinical need, scheduling, and Practice availability.
What if I have an emergency?
Call 911 or go to an appropriate emergency facility. Healow is not for emergencies.

It is intended for patients who are appropriately self-pay at Enrique Fernandez MD PA. Self-pay eligibility is verified at enrollment and renewal.
Can I have health insurance and still participate?
Possibly. The program is based on self-pay eligibility at EFMD, not simply on whether you have an insurance card. Patients without insurance are eligible. A patient with commercial insurance that EFMD does not participate with may also be eligible after the Practice verifies the coverage and its payer obligations. Patients whose commercial plan is accepted by EFMD are not eligible for this self-pay membership. Medicare, Medicare Advantage, Medicaid, and other coverage with special billing requirements require administrative review before enrollment.
What if I obtain insurance after joining?
Notify the Practice promptly. Obtaining coverage does not automatically mean that you are uninsured or automatically determine eligibility. The Practice will review whether you may continue as a self-pay member under the Agreement, applicable payer requirements, and the Practice's participation status with that plan.
